Offboarding

As your last few days in the Division of Experimental Medicine bring to a close your service to our mission, I want to thank you for your contributions in advancing our research program, building our community, and exemplifying the UCSF PRIDE Values each day. Over the next few days you will be receiving several communications from our Division and Human Resources that will provide you guidance on submitting final documentation and last day procedures. Below I have outlined key pieces of information for your review.

 

In addition to the aforementioned information, your laboratory may have additional actions for you in preparation for your last day, including a final health screening. If you have any questions or need additional information, please do not hesitate to reach out to me, your supervisor, or your HR Generalist, at any time. Step 2: Update your UCPath Profile

Upon separation from employment, you can register for UCPath online access as a former employee once your separation has been finalized in UCPath online. There you can access your earnings statements and W-2 forms for payroll information that UCPath processed. You can also keep your personal contact information up to date.  

  • Before you leave UCSF employment, it is important that you update your personal information (address  phone  ) including your non-business email address   in UCPath online to ensure that you receive all payments and communications. 
  • If UCPath has your non-business email address on file, you will receive a notification email as soon as you are eligible to register for access to the UCPath former employee site. Follow the steps to register and activate your account. You must complete the registration process within the timeframe listed in your email message or you will lose access. More information is available on the UCnet website  
Step 3: Submit Final Timesheet

If applicable, you must submit your final timesheet at least one week prior to your last day in order to process your final paycheck by your last day. You will receive notice from Human Resources to complete this within the two weeks prior to your last day.

Step 4: Return UCSF and ZSFG ID Badges

On your last day, return your UCSF and ZSFG ID Badges to your Lab Manager.

Step 5: Return DEM Key(s) and/or Fob

On your last day, return your key(s) and/or fob to your Lab Manager or dropping them off in the DEM Admin Office (Room 605).

Step 6: Return Computer and Equipment

If applicable, return your assigned computer and any other equipment to your Lab Manager or designee prior to, or on, your last day. Ensure that your files are saved on your Lab’s shared network folder prior to your departure.

Step 7: Surrender Lab Notebook(s)

You must surrender your lab notebook(s) to your PI, Lab Manager, or another designee prior to your departure. If you maintain an electronic notebook(s), your access will end following your last day. These records must be kept on file in custody of the Division in accordance with funding requirements.

Step 8: Obtain Final Paycheck

If applicable, your final paycheck will be deposited directly to your bank account(s) based on your UCPath distributions.

Step 9: Information from Human Resources

Your Human Resources Generalist will provide you with an e-mail that details your vacation leave payout, UC benefits coverage, FAQ guide, UC COBRA benefits, retirement account information, and tax information. Save this e-mail (somewhere other than your UCSF e-mail) and review the contents and links to inform yourself of pertinent information.
