Door Access with DPH Badge
The administrative officer will help provision access to unit/doors throughout the ZSFG campus. Each request will include the DPH badge number and area access needed. Request requests are submitted via email to [email protected]. All requests will come from the DHM administrative officer and CC the Division Manager.
Email Distribution Lists for DHM
DHM maintains a set of email distribution lists for cross-sections of people in our Division, which are as follows:

Includes all faculty, staff, advanced practice providers, moonlighters in all services. You use this list when you want to email the entire division. - [email protected]
Includes faculty who attend on FIS inpatient service only. You use this list when you want to email providers and staff who support FIS only. - [email protected]
Includes only faculty and the Division Manager for all clinical services within the Division. You use this list when you want to email only the faculty in the Division. - [email protected]
Includes physicians, both in and outside of our Division, who work as moonlighters to fill schedule openings for FIS. You use this list when you have a clinical shift(s) you need to give up (not swap).
Emergency Contact Form
Our Division maintains a database of emergency contact information for every person in our Division through our Emergency Contact Form. This information is for emergency use within the Division and is submitted to UCSF and ZSFG as a part of the annual Emergency Action Plan.
Lost Key(s) and ID Badges
The division provides you with your initial key(s) and ID badges free of charge. If you lose your key(s) and/or ID badges, you are responsible for the cost of replacing them.
- Key - $25.00, payable by check to the "UC Regents" and given to the DEM Admin team for processing.
- UCSF ID Badge = $0.00, as cost is covered by UCSF.
- ZSFG ID Badge = $20.00, payable by check at the time of your ZSFG ID badge replacement visit.

Records Retention Information

Server Link for DHM
The Division maintains our information on the UCSF server at: //som014.som.ucsf.edu/SFGH$/Medicine/CentralAdmin/MedDivHospMed. Contact the Division Manager to request access. This is predominately used for administrative staff within the Division for day-to-day operations. Faculty should utilize Box or OneDrive for data storage needs.
Travel Policies
In preparing for university related to travel, please review the UC Travel Policies. When traveling on business, it is the responsibility of the traveler to understand UCSF travel and entertainment policies in order to be reimbursed in a timely manner and understand which expenses will and will not be covered by the University. ZSFG Safety Escort
The ZSFG Sheriff’s Office provides 24/7 personal safety escorts within ½-mile radius of the hospital campus, including 24th Street BART.
- Request an escort, call 628-206-8063 (x68063)
- Provide a 20-minute lead time for escort appointments
- If wait exceeds 20-minutes, contact the AOD (6-3519) or the Watch Commander
- Call 911 to report a crime being committed
- Call the Sheriff’s Operations Center(628) 206-4911(x64911) to report a security emergency
